NHS Lanarkshire offers a self referral Podiatry (Chriopody) service which is free to all patients.  If you require a Podiatrist all you need to do is simply fill in the form from the link below and send it to the relevant service which is based nearest to your home address.

Please ensure you fill in the form completely as incomplete forms will be returned and this will cause an unecessary delay in you being seen.
